Infectious mononucleosis, commonly known as mono, is a viral infection that frequently affects adolescents and young adults. The most significant concern regarding mono and physical activity is splenic rupture. During a mono infection, the spleen—an organ located in the upper left abdomen—often becomes enlarged (splenomegaly). Because the spleen is responsible for filtering blood, an enlarged spleen is fragile and vulnerable to injury.

If an athlete engages in contact sports, heavy lifting, or activities involving sudden impact, the risk of the spleen rupturing increases. A ruptured spleen is a medical emergency that can cause internal bleeding and may require surgery. Even if you feel energetic, the internal state of your spleen may not have returned to normal.

Medical experts provide specific guidance to ensure the safety of athletes recovering from mono:

  • The Three-Week Rule: Most healthcare providers mandate a minimum of three weeks of rest from the onset of symptoms before considering any return to sports.
  • Symptom Resolution: Before returning, the athlete must be free of fever, pharyngitis, and excessive fatigue.
  • Clinical Assessment: A physician must perform a physical examination to confirm that the spleen is no longer enlarged and that the patient is physically fit for exertion.

Returning to sports too early can have serious consequences. Beyond the risk of splenic rupture, premature exertion can lead to:

  1. Prolonged Symptoms: Pushing your body when it needs energy for healing can extend the duration of fatigue and illness.
  2. Secondary Complications: Early return may increase the risk of developing further issues such as liver inflammation or pneumonia.
